International Care Management

Your Clinical Advocates

To ensure the highest quality care for you, the care management team — made up of experienced nurses and a senior-level physician — will work closely with your international care coordinator and the Johns Hopkins clinical team.

Patient Education

Understanding your medical condition and what to expect is very important. During your visit, a care management nurse can provide educational sessions to answer your questions and enable you and your family to actively participate in your health care.

Clinical Coordination During Inpatient Stay

During an inpatient stay, a care management nurse will visit you frequently. Although your attending physician and his or her team will coordinate the majority of your clinical care, the care management nurse will make sure everything is coordinated properly and in a culturally appropriate manner. The nurse will also make sure your prescriptions are filled and managed appropriately.

Discharge Planning and Follow-Up

The care management nurse will put together discharge plans with other members of the Johns Hopkins team, keeping your unique needs in mind. Before you are discharged, the team will make sure you receive the right information and education, plus available translated materials. The team will also discuss follow-up visits and treatment needs for care in Baltimore or in your home country.

Medical Equipment

The care management team will arrange for you all necessary medical equipment, such as a wheelchair or oxygen.

Home Care and Private Duty Nursing

At your request, the care management team can arrange home care nursing needs after discharge, a private duty nurse or a sitter with medical background, or domestic help services (basic housekeeping/babysitting).

Prescriptions

When you arrive, please give your coordinator a list of your allergies and any medications you are taking, including the dosage and frequency of each. The care management team will work with your coordinator and Johns Hopkins medical staff to make sure your medications are managed properly — especially if you are seeing multiple specialists or have allergies. At your request, your prescriptions can be delivered to your local hotel or apartment-simply give your coordinator advance notice.

Medical Transportation

If you cannot fly on a commercial flight, the care management team can arrange medical transport to and from Johns Hopkins, as well as transportation to and from airports and special clinical needs, such as oxygen.
